forbidden 404

On my forum, I have my mod who has occasional problems with the below error when accessing the site:

Forbidden


You don't have permission to access / on this server.


Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

The strange part is her IP is never banned by APF and the site is always up live when this happens to her

How can I trace this issue? Is it possible it is her ISP?
