APF stopping Exim from sending mail

ever since I setup APF on my VPS, Exim has been queuing all mail received and generaed locally. I didn't find this out until Exim was creating enough queue runners to bring the house down.

After stopping APF, the queue was able to drain. I've tried disabling all the optional filter settings, egress filtering is disabled, ingress filtering is:
IG_TCP_CPORTS="21,22,25,53,80,110,143,443,2222,3306,4643,50000_51000"

The log is telling me that the connection keeps timing out when APF is in place:

So what might be the cause?
